Hours:
Summer hours are by appointment only. We check our messages daily, Monday-Friday. Non-urgent calls may not be returned until our next day in office, however. Please call 503-876-8564 or 503-876-8562 and leave a message if you wish to be seen.
Medication refills during the summer: please call your pharmacy and have them fax a refill request to our office at least 1 week before your need your refill. If you need a birth control refill, you will need an appointment.
If it is an emergency, call 911 or go to the Emergency Room. If it is urgent and Jacqui is your primary care provider, you can contact her on her personal cell phone at 503-435-8920. Please be respectful and do not use this number unless you need to speak with her on an urgent basis.
